2. IMPORTANT!: Select the dropdown under "Plan Type" at the top of the page and select "IL Medicaid" then click "GO".
3. Under "Quick Actions" fill the patient's last name or Member ID if you have it, then their date of birth. Select the dropdown under "Selection Action Type" and choose "View Eligibility & Patient Information"
4. Then click "Submit"
5. On the "Overview" screen check the patient's eligibility at the top of the screen.
6. If the patient is eligible go ahead and print this page, otherwise make a note on the patient's chart in Rev.
